Payment
Cash, check, ACH and credit or debit cards are accepted for payment. A valid form of payment must remain on file at all times, and your appointment may be charged before, during or after your appointment. If an occasion arises where services do not end up being rendered, a refund is immediately provided.
Insurance
Most major insurances accepted, however please note, your specific plan may alter your allowable services. Some of our insurances we are paneled with include, but are not limited to: Aetna, Cigna, United, Optum, Oscar, Oxford, UMR, Meritain. BCBS or FL Blue are NOT accepted. Medicaid and Medicare are also NOT accepted. Please contact office at (561) 760-1633 to discuss approved carriers as well as coverage.
